Appointments can be made by calling our Appointments and Information line at 901-381-4MOG (4664) between the hours of 8:00 a.m. and 5:00 p.m. Monday through Friday and asking for an appointment. After hours, a message can be left with the answering service and your call will be returned during the next business day.

New appointments: In order to expedite your check-in and minimize waiting times, we ask that new patients arrive at the office 30 minutes prior to their appointment. Please fill out the appropriate "Patient Information" forms below and bring them with you on the day of your appointment. This will save you time when you arrive for your appointment.

Appointment cancellations: We request that appointments be cancelled at least 24 hours in advance. When an emergency or illness arises, we will do our best to reschedule your visit as promptly as possible.

Waiting times: We strive to minimize any wait on the part of our patients. Occasionally emergencies do arise, and you will be informed promptly of any delay in your appointment. We ask that our patients contribute to efficient patient flow by being on time for their appointments and by bringing all requested material with them.
