The FastTrack referral program offers referring physicians the ease of access with one number, one form, and one person to handle their patients’ appointments. This allows for patients to see the specialist earlier, and therefore begin the healing process sooner. The FastTrack referral program has a dedicated number, 381-4MOG, so referrals can be streamlined.

It is appropriate to contact the Doctor’s Assistant with employment report request and/or questions for the Doctor. All other requests for medical records, MRI reports, or PT notes should be directed to our Medical Records Department.

Medical Records

In order to better serve our patients and employers, MOG now offers direct access to our Medical Records Department. You may request medical records, work notes, MRI reports, and physical therapy notes. There are several ways to request records and information from this department. Employment reports are faxed by the doctor’s assistant as the patient leaves the office. All other dictation will be faxed within 24 hours.
